As a patient, family member or caregiver, you may want more information about a healthcare topic or have specific questions about Midwest CareCenter programs. We‘ve compiled much of what you may need here, and we’re always available to respond to calls or e-mail inquiries, if there’s something you don’t see answered on this website. Some of our resources for patients and families include:

Advance directives. Advance directives are oral or written statements that ensure you receive the care you want if you ever lose the ability to make healthcare decisions for yourself. This is a description of each type of document you may want to consider.

Ask our physicians. Questions often arise outside the doctor’s office. You may have a general question or a specific concern about your own health or that of a loved one. Here, our trained and caring specialists respond to specific questions that have been asked by patients and their families.

Blogs and forums. You may be interested in reading blogs or participating in “conversations” about hospice, palliative care or related topics. Here are a few places to start.

Choosing a private-duty caregiver. Selecting an agency to provide personal care services is a private and emotional issue. Here is a brief checklist of the most important questions to ask.

Community outreach & speakers. We have specially-trained staff and volunteer presenters who speak about advance care planning, palliative and end-of-life care, home health safety and other issues to groups of all sizes.

Grief support groups. Midwest CareCenter offers grief support groups that provide a supportive and confidential atmosphere for adults, children and teens who have experienced a loss.

Recommended reading. Midwest CareCenter publishes a number of newsletters and brochures that you may download or request. You may also want to read more on particular issues that you and your family are facing. Here is a list of titles that we recommend.
