HIPAA Notice of Privacy Practices
As a healthcare organization, Midwest Palliative & Hospice CareCenter (Midwest CareCenter) is required to make certain information about privacy available to its patients. Our HIPAA Notice of Privacy Practices can be found here. If we change our privacy practices, we will immediately update our HIPAA Notice of Privacy Practices and make the updated version available through the above link. Any protected health information collected prior to the change will be protected by the policy that was in place at the time the information was submitted.
Website Privacy Practices
Midwest CareCenter respects the privacy of our site visitors and donors. We realize that visitors may be concerned about the information they provide to us when making inquiries and/or donations as well as how that information is treated.
Collected Information
We collect and store the following information about site visitors: domain name from which you access the Internet, the date and time you access our website, and the Internet address of the site from which you linked to our site. We do not track visitors through the use of cookies* and we do not sell or rent the information you provide to us to third parties. (For example, Midwest CareCenter does not partner with or have special relationships with any ad server companies.) Donations, Online Profiles & Contact Form
Midwest CareCenter takes every precaution to protect users' information that is submitted during any transaction or communication. When users submit sensitive information, such as name, address, e-mail address or credit card number, that information is treated as highly confidential. We protect all confidential information from disclosure to the full extent permitted by law. We may use your contact information to send further information about Midwest CareCenter or to contact you when necessary. You can always opt out of receiving future mailings at any time. (See the "Opt Out" section below.)
User IDs & Passwords
Each individual who registers to use the Midwest CareCenter online services receives a unique user ID and password. Users who elect to register on the site will use their e-mail address as their user ID and may choose their own passwords. The user password is the private property of the registered user. For reasons of security and privacy, we do not give out passwords to anyone. If you forget your password, you can request that your account information be re-sent to the e-mail address you originally provided during registration. (Note that this is an automated process, and no employee of Midwest CareCenter has access to this information.)

This site does not host or receive funding from advertising or from the display of commercial content. Funding for www.carecenter.org is covered by the annual operating budger of Midwest Palliative & Hospice CareCenter.
