Midwest CareCenter is a nonprofit, community-based provider of hospice care, palliative care services and grief support. Medicare, Medicaid and many private insurers offer benefits that cover our services. If you do not have insurance or the ability to pay for services directly, Midwest CareCenter will waive or reduce fees as appropriate. We offer financial assistance to all qualified patients through our Charitable and Subsidized Care Program.

How do I apply for financial assistance?

A Midwest CareCenter representative will talk to you about your need for financial assistance. If you think you need help paying for services, please ask your representative for a Financial Assistance Application form. If you need help completing the application, our staff can help you.

At the admission visit, your Midwest CareCenter representative will ask for and verify any current Medicare, Medicaid and private insurance information you have. Midwest CareCenter requires all patients who are potentially qualified for financial assistance to apply for Medicaid benefits as part of the financial assistance process. However, your admission to our care programs will not be delayed by your search for financial assistance. If you are medically eligible to receive our care services, you will be enrolled and begin to receive services immediately.

How much assistance can I receive?

Once your application is received by our billing office, we will determine your eligibility for financial assistance. Discounts are based on the current federal poverty guidelines for your family size compared with your household income and assets. Care is free for those who earn 150% or less than the federal guidelines. For those who earn more than 150% of the federal guidelines, discounts will range from 25% to 75% off regular fees. We will notify you of your discounted fee in writing. If fees after discount are more than $5,000, or there are special needs or circumstances that affect your current financial situation, additional discounts may be applied. For items not covered under our financial assistance program, we can assist with locating community resources to help you pay for them.

What if my circumstances change?

If you are not eligible for financial assistance at the time of your admission but your circumstances change, please call our billing office or ask a member your care team for the application form you need to apply or reapply for the program. An application for financial assistance can be submitted at any time during your care. The form must be filled out completely, and you may be asked to provide supporting documentation.
